When is payment due?

Payment or co-payment is due at the time of service. We participate with most major insurance companies and our agreement with them requires that we collect your co-payment. Services received that are excluded or not covered by your insurance company are your financial responsibility.

What insurances do you accept?

We participate in most health insurance and managed care plans. If you have questions about our fees and your insurance coverage, call us at 216-450-1613. It is your obligation to inform ARC of any changes to your insurance coverage.

What is the policy for cancelling or rescheduling an appointment?

Appointments must be cancelled or rescheduled at least 24 hours in advance. Please refer to our No-Show Policy for more details.

How early do I need to arrive for my scheduled appointment?

New patients should arrive 30 minutes prior to their appointment time to allow themselves time to complete our paperwork. Filling out the forms ahead of your appointment will speed up the process upon arrival. Established patients should arrive 10 minutes prior to their appointment to allow for any front desk traffic, scheduling, and billing issues to be taken care of. Patients not doing so may run into the start of their appointment, resulting in a shorter session.

How do I obtain my medical record?

All requests for medical records must be made in writing. Requests for medical records not from the individual client must include a fully completed Consent for Release of Information form. Fees may be associated with requests.
